Send Feedback

Send Feedback is on the menu sheet, not a row in the main menu list. The tooltip is "Send Feedback". Vessel owners and marine professionals both have it. The owner is FeedbackMenuWidget, opened from feedback_flow.dart.
The sheet title is "Send Feedback". The line under it is "Help us improve by reporting bugs or requesting features".
Three choices:

Pick one to open the form for that type. Close the sheet with the close control if you are not sending anything.
